Important information – Upcoming changes related to your service provider at Victoria and Five Elms Medical Centre

As of 30 September 2025, East London NHS Foundation Trust (ELFT) will no longer hold the contract for Victoria and Five Elms Medical Centre. A temporary provider will be put in place to provide the same services as your current GP and we expect you to continue to see the same care teams, as staff working at the practice should transfer with the services. This means that as a registered patient you should not see any interruption in the services provided.

Access to medical records

The practice is registered with the Information Commissioner’s Office. We comply with the Data Protection Act 2018.

We manage requests for access to medical records according to the Act.

You can usually access your records via the Evergreen Life app.

Use your smartphone to scan the QR code or search for Evergreen Life PHR in your app store.

You can use Evergreen Life to start building your Personal Health Record to understand how to be as well as you can be and get support if you are ever ill.

The easiest way to connect your records is with an NHS Login – if you have one already you can use it to register, or create one as part of the process.

If you cannot use an NHS Login, you might need to provide photographic identification to our receptionists who will issue you with an account ID and link to sign up.  If you have had access to online services in the past you may already have done this.

Summary Care Record

If you’re registered with a GP surgery you’ll have a Summary Care Record, unless you’ve chosen not to have one. It contains basic information including your allergies and medicines. It also includes any previous reactions you’ve had to medicine.

Having this information in one place makes it easier for healthcare staff to treat you in an emergency, or when the practice is closed.

GP2GP

GP2GP transfers electronic health records between a patient’s old and new practices. It is a direct, secure and quick way to send your medical records when you change GPs.

Your data matters to the NHS

Your health records contain a type of data called confidential patient information. This data can help with research and planning.

You can choose if you want to share your data. You can also choose for someone else, such as your children under 13.

Your choice will only apply to the health and care system in England. It will not apply to services you use in Scotland, Wales or Northern Ireland.

Need a little help?

To help you get started with the Evergreen Life app and creating your account there are a series of help articles available.  Linking your account to your GP record will enable the full functionality of the app including records access and repeat prescriptions as well as enabling you to make requests for help online.

You can link using an NHS ID which is the easiest method for both you and the practice team, or by using linkage codes supplied by the practice.

We’ve also created a walk-through video to show the steps of using the app for the first time.
